











To help facilitate your exchange experience, a number of forms must be signed
and/or completed by you, your host university, your home institution, or your
program sponsor during the course of your program. Some of these forms can be
downloaded in PDF format. To open and print these documents, you must have the
Adobe Acrobat Reader software. If your computer does not have a copy of the
program, you may download a free copy by clicking on the "Get Acrobat Reader"
icon to the right. Once you download Adobe Acrobat, click on the appropriate link to
access your forms.
Forms
you must complete before your departure:
Please download the appropriate language version, if available. Unless noted
otherwise, these forms should be submitted by January 15. Please contact your
Fulbright Commission or Public Affairs Sections of the United States Embassy
to confirm whether the forms should be submitted to them or sent directly to
LASPAU. It is not necessary to send any of the documents by express mail or
fax. If you are sending forms to LASPAU, please send them to the following address:

- Acceptance of Scholarship Form: Fill out this form, indicating whether
or not you accept the scholarship, and sign it: English

- Fulbright-OAS Terms and Conditions of Grant: This document details
your rights and responsibilities as a Fulbright grantee. Sign this form as
proof that you have read the terms and that you are in agreement with them:

- Release of Application and Academic Information Form: Sign this form
to authorize LASPAU to request information from your U.S. host institution
pertaining to the admissions process and your subsequent academic progress:

- Certificate of Health: This form is to be filled out by a physician
in your home country and returned to the Fulbright Commission or Public Affairs
Section of the U.S. Embassy by January 15, 2005. Make a copy of this form
as you will need it to enroll at your U.S. university: English

- Dependent Visa Information Form: This form allows LASPAU to initiate
the processing of J-2 Family Visas for the family members who will be joining
you in the United States at the beginning of your academic program. Along
with the requested information on the form, we require documentation confirming
you have funds and insurance coverage for your family during their stay in
the United States. Please fax the completed form and supporting documentation
to LASPAU no later than three months before your family's planned arrival:

Forms you
must complete after your arrival:
- Confirmation of LASPAU Grantee
Presence at Host Institution: Have your international student advisor
or the appropriate official sign this form indicating your date of arrival
at the university, and return the completed form to LASPAU within 15 days
of your arrival. We need this form in order to maintain your legal status
in the United States and issue you further maintenance payments. Click

- Information Sheet: This sheet includes three forms: 1) the Grantee
Information Form provides LASPAU with information about you, your university,
and your home institution; 2) the Program Planning Form provides confirmation
that your academic program meets the requirements of your LASPAU-administered
grant; 3) the Academic Information Release Form authorizes LASPAU to
obtain your academic information from your university in the event of your

Forms you
must complete during your academic program:
- Academic Progress Report:
Twice a year, you will be asked to report electronically on your grades and
enrollment plans. The electronic report will, in turn, be sent to your program
sponsor. You will be notified by email when it is time to fill out this form.

Complete and return this form to LASPAU by mid-April to avoid problems with
tuition coverage or delays in maintenance payments. Click
