What do I do in an emergency?
If your medical condition is an emergency, visit the nearest appropriate medical facility or call the emergency help number in your community. After receiving treatment, call Seven Corners, Inc. to notify the Company of your emergency treatment.
What do I do for a non-emergency illness or injury?
In a non-emergency situation, you have three options for treatment, each with different coverage: (1) utilize the services of your health center (2) Contact customer service 800-683-1427 or call collect: 317-818-2806 and they will give you the name of a physician/facility that is associated with Seven Corners, Inc. These providers usually will bill the provider or facility & will not require upfront payment. – You can also get a response via email at claims@sevencorners.com , or (3) contact a physician/facility outside the Seven Corners network.
How can my Doctor find out about my coverage?
If the doctor has any questions about your insurance protection, ask him or her to call Seven Corners, Inc., which is staffed 24 hours a day (see above phone numbers).
If the doctor or the hospital calls the customer service number, Seven Corners will confirm the coverage in accordance with the insurance terms and conditions. Some providers and hospitals abroad insist on the patient paying the invoice immediately. If this is the case for you, please send or fax the original invoice, a completed claim form, and the receipt indicating that you have paid the bill to the following address.

All claims should be submitted within 3 weeks in order to ensure timely and accurate processing. |
What information do I need to provide when filing a claim?
You must mail or fax the following items to the address listed above:
- Your claim form with all items completed and signed
- All itemized medical and hospital bills
- Receipts if the bill has been paid and you are to be reimbursed
- Reimbursment will be made in U.S. currency & sent to your bank account via wire transfer. If this is desired, please provide the appropriate bank information
