LASPAU must receive this completed report no later than May 30, 2008 (or as soon as your grades are available).

LASPAU collects information about your academic progress twice a year, in January and June. When you are done filling out this form, press submit. The report will automatically be emailed to your LASPAU program advisor as well as to your academic advisor using the email address you provide for him/her below.
